SPRINGFIELD — The Springfield Post of the Ohio State Highway Patrol is investigating a two-vehicle fatal crash that occurred Monday evening at the intersection of state routes 4 and 334 in Moorefield Township, Clark County.
David Antonio, 31, of Springfield, was pronounced dead at the scene after sustaining fatal injuries in the crash, according to the Patrol.
Gavin Dyer, 18, of Urbana, and his passenger, Adisyn Welty, 18, also of Urbana, were transported by ground ambulance to Springfield Regional Medical Center, where they were treated for minor injuries.
Troopers said the crash occurred at approximately 6:34 p.m. Preliminary findings indicate Antonio was driving a 2005 Saturn Ion eastbound on S.R. 334, while Dyer was operating a 2009 Honda Accord southbound on S.R. 4. Investigators reported the Saturn failed to yield at a stop sign at the intersection, entered S.R. 4, and was struck by the Honda. After impact, both vehicles traveled off the east side of the road.
Assisting at the scene were the Clark County Sheriff’s Office, Moorefield Township Fire Department and EMS, the Clark County Coroner’s Office, and Rudds Towing & Auto Recovery.
The intersection was temporarily closed while crews worked the scene and conducted the investigation. The roadway reopened shortly after 8:40 p.m.
The crash remains under investigation.
