CHWP set to open new pharmacy at Bellefontaine facility

Community Health & Wellness Partners (CHWP) announced this week that the opening of its new community pharmacy is set for Monday, Dec. 2. 

Located at their Bellefontaine Community Health Center, 212 E. Columbus Ave., Suite 1, the pharmacy will serve as an extension of the pharmacy services that CHWP has provided for over nine years. 

Since 2015, patients have benefited from medication affordability, education and therapy. CHWP will continue to offer discounted pharmaceutical prices on qualifying medications, now with the additional convenience of an on-site location. Patients can also take advantage of delivery services, either directly to their home within Champaign or Logan County or a more convenient CHWP location for pick up, including one of their school-based health centers. 

“Meeting our patients where they are and providing comprehensive care to all our patients is our driving force. We work hard to remove barriers and make care accessible to anyone and everyone,” said Tara Bair, CHWP president and CEO. “We are the provider of choice for our community, so it only makes sense to further integrate our services with an on-site pharmacy. Because of our facilities and services, healthcare is more accessible, affordable and we can advocate for the wellbeing of our community.”

Services will continue to grow to meet the needs of CHWP patients and the community. Unique to CHWP’s Pharmacies, when a patient visits their provider, they can fill prescriptions in one convenient visit. 

Additional services will include pharmacy consultation, medication box refills, and monitoring of chronic disease management such as heart disease, diabetes and hypertension. 

The CHWP pharmacy team will be made up of two clinical pharmacists, two community (retail) pharmacists and support staff. 

“Offering all these services to our community helps us to remove barriers to care for patients here in Champaign and Logan County,” said Erin Zurlinden, PharmD, CHWP community pharmacy manager. 

“We are excited to meet and learn more about our patients and provide them with the kind of personal, convenient and quality pharmacy service they deserve.” 

Access to their knowledgeable team is growing with the RxLocal app. While patients may continue to reach the pharmacy via phone, the RxLocal app provides patients with direct messaging capabilities, as well as the ability to manage all family prescriptions, request refills and set medicine reminders. 

CHWP’s Community Pharmacy will participate in the 340B discount drug program, transferring those cost savings on to their patients. Contracted pharmacy partnerships with Aries Pharmacy, C&R Pharmacy, Medicine Shoppe and existing community pharmacies in both Champaign and Logan County will also continue with the same benefits as before.
