Beth L. Tevis, 58, of Bellefontaine, was transported by a Bellefontaine squad to Mary Rutan Hospital for treatment of possible injuries after a 4:35 p.m. crash Friday on Main Street at Lake Avenue.

She was southbound in a car that was hit four times in the rear by a van operated by Harold L. Brown, 75, of Cedarville.
Brown told officers of the Bellefontaine Police Department the brakes failed to engage and they found evidence a rear brake line was damaged leaking fluid.
Officers report the Brown vehicle struck the Tevis car and knocked it forward.
The van with it faulty brakes also continued southbound hitting the Tevis car a second. The same sequence of events occurred two more times for a total of four rear end collisions.
Damage was moderate to the vehicles.
Son lodged for threatening mom
Trenton E. Mathews, 39, of 413 E. Sandusky Ave., was charged Sunday with felony domestic violence.
He allegedly threatened his mother, Jacqueline Loy, was a three-foot-long, two-by-four board.
He ran from officers but was found shortly after in the vicinity of Brown Park. He was arrested and lodged in the Logan County Jail.
